HOME BASED SERVICES
The Home Based Services department at Southcentral Foundation provides a wide variety of services delivered at the customer’s home. These meet the needs of infants, children, families and adults of all ages, and include several categories of care—acute, chronic, palliative and hospice. Customers must be SCF–eligible customers who live in the Anchorage area and obtain a referral from a health care provider who is willing to provide ongoing coverage.
Conditions that might qualify people for home-based services include:
- frequent falls or safety issues
- frequent emergency room visits or hospitalizations
- exacerbations of disease processes
- post-surgical recovery
- need for wound or ostomy care
- need for medication management
- rehabilitation issues (e.g., post-stroke)
- medication or treatment compliance issues
- need for assistance in disease/disability management
- end-of-life care needs
The Home Based Services department offers:
- Skilled Nursing services: medication management, wound care, ostomy care, pain management, infusion therapy, case management, education.
- Home Health Aides: assistance with daily activities, range-of-movement, home safety.
- Physical Therapy: in-home “P.T.,”subject to availability of therapist
In addition, the Home Based Services department manages two distinct sub-programs. These are:
- Nutaqsiivik: a home visiting, case management and support program for families expecting a child or parenting a child less than one year of age. It is staffed by community health nurses. Its aim is to reduce the risks and complications of pregnancy and early childhood by providing families access to the support they need to create safe home environments, strengthen their bonds, and move toward self-reliance.
Nutaqsiivik also runs a special program for high-risk children up to the age of two to protect them from RSV (Respiratory Synctial Virus) infections. Given monthly injections of the prophylactic medicine Synagis from October through May, they are largely protected from this disease.
- Palliative Care: a service for customers facing life-limiting illness. Addresses pain and symptom control, anticipatory grieving and bereavement support, infusion therapy, assistance obtaining medical equipment, and ongoing emotional support.
