Waiver Care Coordination is a program that assists eligible customers (Elders or disabled adults) with obtaining a full range of services so they can remain living as independently as possible. Through this program, customers can receive help applying for Medicaid and Waiver screening (for the Alaska CHOICE Medicaid Waiver Program), while also receiving coordination of support services and overall advocacy for their basic needs.


The Alaska CHOICE Medicaid Waiver Program stands for “Community and Home Options to Institutional Care for Everyone,” and it offers alternatives to people who otherwise would have to live in a nursing home.


At SCF, the Waiver Care office is staffed by certified Care Coordinators who assist customers in gaining access to Medicaid Waiver and other desired services. The Care Coordinators assess what services the customer needs, and then develop a plan of care that informs the state which services should be approved. They assist in setting up the services and monitor the customer’s response to them through phone contact and monthly home visits.


Services that a customer might receive through the Waiver Care program include:

  • home delivered meals
  • environmental modifications (wheelchair ramps, bathroom grab-bars, etc.)
  • personal care and chore assistance
  • transportation for appointments, shopping, etc.
  • placement in assisted living arrangements
  • respite care

To qualify for Waiver Care Coordination, a customer must be eligible for Medicaid and meet nursing home level-of-care standards, yet still be able to live safely and comfortably in the community, given the proper help.
